About the TEIP Program Assessment Tool

What type of program is the Program Assessment Tool designed for?

The Program Assessment Tool was originally designed for Heart Health Promotion programs that are already underway. However, the Tool can be used for all community-based, population wide Health Promotion and Chronic Disease Prevention programs no matter the area of concentration.

What does the Program Assessment Tool do?

The information that results from using the survey, assessment, and consensus worksheet helps practitioners identify areas for enhancement in existing health promotion programming.

What does the Program Assessment Tool not do?

The Tool does not identify a program as 'Best Practice', 'Promising Practice' or 'Recommended Practice'. It is also not intended to be used to compare different programs.

How much time will it take to complete a program assessment?

The complete program assessment process can take a number of days. However, users generally feel that the process is worth the investment of time. In addition to identifying needed program improvements, the process also helps with the creation of program documentation that can be used outside the assessment process.

How do I use the Program Assessment Tool?

  • Step 1: Select a program to assess and assign roles for carrying out a program assessment using the Roles Assignment Worksheet6pgs, 124kb
  • Step 2: Prepare a program report by completing the Program Information Survey 6pgs, 124kb and provide any required documentation.
  • Step 3: Identify two to three independent reviewers to assess the program. At least one reviewer should have limited knowledge about the program to provide fresh perspectives and suggestions. The independent reviewers assess the program and provide suggestions for program enhancement by reviewing the program report (i.e. completed Program Information Survey) and applying the criteria found in the Program Assessment Worksheet6pgs, 124kb.
  • Step 4: The reviewers meet to assign final assessment levels and discuss suggestions for program enhancements using the Consensus Summary Worksheet 6pgs, 124kb. The consensus meeting can be held in-person or by teleconference and should be facilitated by a facilitator independent from the review process. Guests could also be invited to provide clarification on program activities.
  • Step 5: Select and implement suggestions, using the Program Enhancement Work Plan 6pgs, 124kb, develop a feasible plan to implement the priority program improvements developed during the consensus meeting.
